I came across this document while doing some research. Starting on page 43, the document summarizes the most commonly violated rules of professional conduct in Alabama between 1992 and 2006. I'm sure each jurisdiction is different, but I thought the list was interesting. The top two most common violations by a large margin were willful neglect (Rule 1.3) and failure to communicate (Rule 1.4).
